I release and hold harmless United Hatzalah and all related parties from any claims. I will meet the $360 fundraising minimum by the deadline, even if I do not participate. I allow United Hatzalah to use my name and likeness for publicity, without compensation. I am 18 or older, or have parent or legal guardian consent.
